Affidavit
Prepare a simple affidavit that is to be signed and sworn to on oath by George Berry (it will be executed on an unknown date in the not too distant future). Your document should be in a form that is ready to sign without the need for further editing. In the affidavit George's signature must be notarized. The notarial certification must comply with the requirements of Chapter 117.05(13) Florida Statutes. In the statute there are a number of notary certification templates that can be used (depending on the circumstances). Choose one of the more simple templates near the bottom.
In the affidavit George should make the following statements.
- that he is the owner of 100% of the shares of Egocentric, Inc., a Florida corporation.
- That he is corporate secretary of Egocentric, Inc.
- That as secretary he is the officer designated to be the keeper of that corporation's records.
- That the document labled "Bylaws" which is attached to this affidavit is a true and accurate copy of the current bylaws of Egocentric, Inc. and that those bylaws were properly adopted by that corporation's board of directors.
Note: Don't worry about attaching the bylaws. The bylaws will be attached to the affidavit at a later date prior to its signture.
